Three-dimensional (3D) mesenchymal stem cell (MSC) cultures are revolutionizing skin regeneration by enhancing the regenerative potential of MSCs through improved cell-cell interactions and extracellular matrix (ECM) production. Compared to traditional 2D cultures, 3D systems better mimic the in vivo microenvironment, leading to increased secretion of bioactive factors and extracellular vesicles (EVs). These MSC-derived EVs carry essential growth factors, RNAs, and proteins that promote wound healing, angiogenesis, and collagen synthesis while reducing inflammation and fibrosis. The combination of 3D MSC culture and EV-based therapy offers a promising, cell-free approach to treating chronic wounds, burns, and aging skin. Future research should focus on optimizing 3D culture conditions, EV isolation techniques, and delivery methods to maximize therapeutic efficacy.
